We do not sell Personal Information that you enter on our website. We collect information from our contact forms to be able to contact the person who requested contact by completing the form. Our company is hosted on the Wix.com platform. Wix.com provides us with the online platform that allows us to advertise and communicate our services to you. When you contact us through our company website, Wix.com emails your information to us. Your data may be stored through Wix.com’s data storage, databases and the general Wix.com applications. They store your data on secure servers behind a firewall.

​If you complete our contact form, we may contact you by the email address or phone number you provide us for the purpose of answering your questions about our services and to schedule an appointment. If you choose not to be established as a patient with us, we will not contact you again.

 

Email is not considered to be a confidential form of communication due to risks of hacking and other unauthorized access by a third party. If you share your email address with us on our contact form, you agree to allow us to email you and accept the risks in doing so. If you wish to contact us without the risk of your information being intercepted by a third party, please do not complete our online contact form and call us instead at (919) 600-4906.

Electronic Intake Forms

 

If you make an appointment with us, you have the option of completing our new patient intake forms via electronic means. We utilize a HIPAA-compliant company for these forms, which record the date and time you completed the form as well as your IP address. We collect and store information for the purpose of treatment, payment, and healthcare operations and safeguard your information per HIPAA standards. You are not required to complete these forms electronically and may request paper forms instead.

Payments

 

All direct payment gateways offered on this website and used by our company adhere to the standards set by PCI-DSS as managed by the PCI Security Standards Council, which is a joint effort of brands like Visa, MasterCard, American Express and Discover. PCI-DSS requirements help ensure the secure handling of credit card information by our company and its service providers.

 

Minors

 

Although Etheridge Psychology provides mental health services to children and adolescents, our website is intended for a general audience and is not targeted to minor children (under age 18). We do not knowingly collect Personal Information from minors.
